In consultation with your dissertation advisor and upon final acceptance and approval of your dissertation by your committee, you may complete this form and submit it along with a PDF of your work. This contains a release for the binding of your dissertation, a copy for you, one for your department, and one to be added to the Guggenheim Library Archives. This may also include uploading to the ProQuest Theses & Dissertations database.
